We have a formal process for releasing medical records because of our commitment to protect your medical information. Visit myCare Patient Portal for visit summaries and test results.

Please visit and sign-in to the myCare Patient Portal to view your most recent:

  • Visit summaries of past appointments or hospital inpatient stays
  • Test results
  • Medications

If you have an activation code please Sign Up for access or if you are new to myCare please request an Activation Code for access.

Obtaining Copies of your Medical Records

To protect your privacy, we require that you fill out and return an authorization form in order to release your medical records — learn more about your rights as a patient and the use and disclosure of your medical information in our Notice of Privacy Practices.

To get a copy of your medical records from El Camino Hospital or to have your records sent to another healthcare facility, please download and complete an authorization form.

There's no fee for records released to a doctor's office or a medical facility. For personal requests for records, please call our medical records department for fee information — representatives can also answer questions about the authorization form or our medical records release policies:

Completing an Authorization

The Health Insurance Portability and Accountability Act (HIPAA) requires hospitals to obtain an original, complete and properly executed authorization form in order to provide a copy of a patient's medical record to a patient or anyone he or she designates.

In order to release medical records:

  • Only the patient, parent or legal guardian, or the patient's legal healthcare representative can sign the form to release medical records.
  • The authorization form must be legible and complete — illegible or incomplete forms will be returned.
  • Requests for information regarding AIDS/HIV, mental health or chemical dependency must be specifically initialed on the form.

Processing a Request

Records are usually available within seven to 10 business days from the time the request is received.

If want to pick up your medical records in person once they’re available, you must bring a photo ID with you. To arrange for someone else to pick up the documents for you, please contact the medical records department at 650-988-7462 (Mountain View) or 408-866-4094 (Los Gatos).

Please note that we don’t keep or maintain birth certificates. The Public Health Department officially records births in Santa Clara County:
