Have you received care through the RotaCare Clinic located on El Camino Hospital’s Mountain View Campus? We have important news for you.
The RotaCare Clinic on our Mountain View hospital campus will close at the end of May 2016, but you can continue to receive excellent health care services from one of three local clinics that are part of the MayView Community Health Center.
El Camino Healthcare District and Hospital remain committed to serving your health care needs and the needs of all low income community members. We’ve made changes which we believe will improve your access to comprehensive, high quality health care close to home and during extended hours. In January, the El Camino Healthcare District Board approved funding for a new, expanded partnership with MayView Community Health Center. MayView is a respected community health organization which provides health care for individuals and families, including those who have no health coverage, as well as those who have Medi-Cal coverage, or who may be eligible for Medi-Cal and other health and social services.
RotaCare patients who go to MayView will now have a dedicated doctor or nurse practitioner, and a place where the staff will know you and can help you get the care you need. In addition to comprehensive primary care services, MayView provides perinatal care, chronic disease management, health education, immunizations, mental health services, care coordination and more. All of these services are provided to all underinsured and uninsured community members, regardless of ability to pay. If you cannot afford to pay, you services will be provided free of charge.
For RotaCare Patients
The RotaCare Clinic is evaluating and providing primary care services to existing patients through the end of May.
If you are a RotaCare patient and visited the clinic after July 1, 2015, our staff will match you with a doctor or nurse practitioner at one of three MayView clinics located in Mountain View, Sunnyvale and Palo Alto. You will receive a packet which includes information about the transition, your recommended MayView doctor or other health care provider, provider photo and short bio, contact information for MayView’s dedicated RotaCare liaison and information on how to access your medical records. If another health care provider is more appropriate because of your health condition, we can help with that also. If you have not already been in contact with the clinic staff regarding the transition of your care, please call 650-988-8202.
Patients who visited the clinic after January 1, 2015 will be notified of the transition and given instructions about how to obtain a copy of your medical records. If you have not already obtained a copy of your medical records, please call 650-988-8202.
