Medical Records

El Camino Hospital is committed to protecting your medical information. For information about your rights and the obligations we have regarding the use and disclosure of your medical information, please see our Notice of Privacy Practices.

To obtain a copy of your medical records from El Camino Hospital, please download and complete an Authorization to Release Medical Information form located on the right side of this page. You may also use this form to have your medical records sent to another medical facility.

Completing An Authorization

The Health Insurance Portability and Accountability Act, also known as HIPAA, requires hospitals to obtain an original, complete and properly executed Authorization for Disclosure of Health Information form in order to provide a copy of a patient's medical record to a patient or anyone he or she designates.

Only the patient, parent/legal guardian or the patient's legal Health Care Representative can sign the form to release medical records.

The authorization form must be legible and complete. Authorizations that are illegible or incomplete will be returned. Requests for information regarding any of the following items must also be specifically initialed on the form:

  • AIDS/HIV
  • Mental Health
  • Chemical Dependency

Records released to a physician's office or a medical facility are free of charge. 

Processing A Request

Records are usually available within 7 to 10 business days from the time the request is received.

If you are interested in picking up your medical records in person, when they become available, please note this at the top of the request form. If you plan to come to the Medical Records department to pick up a request that you have submitted, you must have a photo ID with you. To arrange for another individual to pick up the documents for you, please contact the appropriate Medical Records department for instructions. (In Mountain View at 650-988-7462; in Los Gatos at 408-866-4094).

Once your Authorization for Disclosure of Health Information form is completed, you may submit your request by mail, fax or in person.

Birth Certificates

Births in Santa Clara County are officially recorded by the Public Health Department. You can get copies of birth certificates from the Vital Records department. The Vital Records department provides birth certificates for the current and one previous year only. Older certificates are available from the Santa Clara County Clerk-Recorder or call 408-299-2481.

El Camino Hospital does not keep or maintain birth certificates.

At our Mountain View campus, the Medical Records Information desk is available at 650-988-7462, Monday through Friday, from 8:00 a.m. to 4:30 p.m. At our Los Gatos campus, the Medical Records Information desk is available at 408-866-4094, Monday through Friday, from 7:00 a.m. to 4:30 p.m.

Please note that your medical records do not contain any billing information. Visit the Billing & Insurance section of this web site for information about your hospital bill.

Film Request Form

If you've scheduled an appointment to get a mammogram, it's important that you provide us with previous mammograms so that we can read your new mammogram properly. To authorize us to request your mammograms from other healthcare providers, please use this form.
