Bay Area Healthcare Organizations Salute Veterans with Advance Care Planning Health and Resource Fair

Sunnyvale, CA - October 16, 2012 -- Local healthcare organizations are joining together to present a health and resource fair, Advance Care Planning with a Salute to Veterans. It will be held Wednesday, November 14, 2012, 11 am to 2 pm, in the El Camino Hospital downstairs piano lobby at 2500 Grant Road, Mountain View, CA 94040.

The event is scheduled for the week of Veterans Day and will highlight some of the special needs of veterans. Co-hosted by Pathways Home Health & Hospice, El Camino Hospital and the VA Palo Alto Health Care System, the resource event is open to the general public and free of charge. No reservations needed.

Representatives from each of the sponsoring healthcare organizations will be present to answer questions about healthcare concerns including:

  • Advance directives and P.O.L.S.T.
  • Resources for veterans
  • Post-traumatic stress disorder
  • Hospice and palliative care
  • Home health care

The VA Palo Alto Health Care System

The VA Palo Alto Health Care System (VAPAHCS) consists of three inpatient facilities located at Palo Alto, Menlo Park, and Livermore, plus seven outpatient clinics in San Jose, Capitola, Monterey, Stockton, Modesto, Sonora, and Fremont.
