When you come to the Centerl, remember to bring the following:

• The orders and instructions given you by the Urologist for admission.
   
• X-rays and tests done at other facilities in preparation for your ESWL treatment.
   
• A list of all the prescription and "over-the-counter" medications that you are taking along with dosage and the name of the prescribing doctor for review by your anesthesiologist.
   
• Your insurance card and related insurance forms.
   
• Personal items that you may need during your anticipated stay. If you are being treated as an outpatient you will need a family member or friend to drive you home.
