El Camino Hospital provides a comprehensive approach to senior health both inside the hospital and beyond its walls. With the Senior Health Program, our patient-centered approach means that seniors not only have access to the primary care services outlined below, but they also have access to senior resources like the Health Library and Resource Center, RoadRunners and Lifeline services, Advanced Health Care Directive assistance and much more.

Our compassionate clinicians are dedicated to providing care unique to each individual and situation. Services for the Senior Health Program are highly coordinated, electronically accessible and multi-disciplinary. Access to patient records through a password-protected patient portal enables patients, clinicians and physicians to collaborate in real-time, streamlining the care process and program with your best interests in mind.

  • senior-focused wellness and prevention care
  • chronic disease management, acute illness and diagnosis, injury diagnosis and care
  • nutritional assessments and education
  • medication counseling
  • social and psychotherapy evaluations

Care coordination may occur across multiple disciplines including nutritionists, geriatric case workers, laboratory staff, pharmacists, financial counselors and more. The Senior Health Program accepts only Medicare Part B fee for service, commercial secondary insurance and cash for services.
