Senior Health Center services are highly coordinated, electronically accessible and multi-disciplinary. Access to patient records through a password-protected patient portal enables patients, clinicians and physicians to collaborate in real-time, streamlining the care process and program with your best interests in mind.

Primary care services include:

• senior-focused wellness and prevention care
• chronic disease management, acute illness and diagnosis, injury diagnosis and care
• nutritional assessments and education
• medication counseling
• social and psychotherapy evaluations

Care coordination may occur across multiple disciplines including nutritionists, geriatric case workers, laboratory staff, pharmacists, financial counselors and more. The Senior Health Center accepts only Medicare Part B fee for service, commercial secondary insurance and cash for services.
