Our senior-care focused consultants are here to help you and your family members deal with the many challenges of aging, whether they are physical, mental, emotional or social. We listen to your questions and concerns, and then, together, we develop an action plan, drawing upon a broad array of information and referral resources, community services and educational programs, regardless of where you or your family members live.

The outpatient Senior Health Center addresses the issue of reduced access to care and resources for our senior community. The center fills the need of seniors to find a compassionate, quality physician who will accept them after 65 or on Medicare. Based on the newest model of patient centered care, the "medical home," the Senior Health Center is a health care resource providing a continuum of care from wellness and prevention to post-hospital stay.
