The staff and management at the Rehabilitation Center are committed to excellence, integrity, teamwork and making rehabilitation fun.
Our team consists of rehabilitation nurses, physical, occupational and physical therapists, as well as case managers, most of whom have been working in rehabilitation therapy in the West Valley/San Jose community for many years.
24-hour nursing care
Our nurses are trained and experienced in providing the special care needed for persons with physical disabilities and chronic illness. In addition to providing direct patient care, the rehabilitation nurse will provide education about rehabilitation care as well as act as the patient’s and family’s advocate.
Occupational therapy

After assessing the patient’s functional limitations, the occupational therapists design a treatment plan that emphasizes daily living skills so that the patient can regain levels of independence, work readiness and ability to perform leisure activities.
Physical therapy

The physical therapist, after assessing the patient’s strength and capabilities, implements a treatment plan to improve the patient’s functioning and mobility. The physician therapist also teaches patients how to use aids such as prostheses, walking aids and wheelchairs.
Speech therapy

Speech pathologists assess the ability of the patient to communicate and treats verbal, written disorders. Speech pathologists also direct treatment plans for patients with dysphagia (swallowing problems).
