The new Rehabilitation Center at El Camino Hospital Los Gatos, offers physicians, care coordinators and discharge planners a locally managed, high-quality referral option for patients in need of intensive rehabilitation therapy in order to recovery from illness or injury. Our comprehensive program features a full complement of aggressive inpatient rehabilitation services tailored to each patient based on his or her needs, tolerance for therapy and desired outcomes. The program is under the direction of an experienced physician, who is specially trained in physical and rehabilitation medicine. Our rehabilitation nurses, physical and occupational therapists, and speech-language pathologists bring a wealth of experience in restoring basic capacities necessary for day-to-day functioning.
The facility, one of only three serving patients in the West Valley/San Jose area, offers a pleasant, home-like environment. Our spacious gym area offers floor to ceiling garden views. Patients eat all three meals in a cheerful, sun-filled dining room. A separate apartment offers a place where patients can practice the skills needed to return to home and as much independence as possible.

Types of patients
The Rehabilitation Center offers comprehensive intervention for patients affected by, but not limited to, the following:
- Stroke

- Brain injury
- Multiple sclerosis, Parkinson’s Disease
- Spinal cord injury
- Amputations
- Polyneuropathy
- Guillain Barre
- Hip fractures
- Degenerative spine disorders
- Joint replacements
- Arthritis
- Decline in function
Common conditions:
- Balance and coordination impairments
- Weakness or limited motion in arms, legs or trunk
- Difficulty swallowing
- Inability to move in bed or from one place to another
- Inability to perform activities of daily living such as eating, grooming, dressing, bathing and homemaking
- Cognitive/perceptual deficits or speech-language problems in conjunction with a physical limitation
- Inability to work or function in the community due to a physical impairment
Referral guidelines
- Demonstrates need for multidisciplinary rehabilitation program of at least two disciplines (includes rehab nursing, physical, occupational and/or speech therapy, and caregiver training)
- Able to participate in multi-disciplinary therapy program
- Requires 24-hr nursing care
- Community discharge plan home (home, home with assistance, home with day-program, board and care or assisted living)
- Adult – 18 years of age
- Expected rehabilitation potential – functional improvement
- Medically stable
