Thank you for your interest in joining the El Camino Hospital Medical Staff. To begin the process, please download and complete the PDF Pre-application Questionnaire - fax the questionnaire to our Credentialing Coordinator as directed.
Once the pre-application is received and basic qualifications confirmed, an online application for medical staff membership will be sent via the e-mail address provided by the practitioner. As part of the application process, you will be asked to acknowledge that you have been furnished with a copy of the Medical Staff Bylaws of El Camino Hospital and agree to be bound by the Bylaws and Rules/Regulations of the Medical Staff.
