To take advantage of volunteering opportunities at El Camino Hospital, adult Auxiliary volunteers need to complete an application form (see below), have a personal interview with an Auxiliary Placement Counselor, and then pass a background check and a health check, both provided by the hospital at no charge to the applicant. Adult membership requires an annual $20 fee and a commitment of 12 hours per month, for a minimum of six months. Most Auxiliary volunteers also wear a uniform, part of which may be purchased in the Auxiliary office.

To apply as an adult volunteer for the Auxiliary at either El Camino Hospital Mountain View or El Camino Hospital Los Gatos, you may contact the Auxiliary office at 650-940-7214 to request that an application be mailed to you, or you may fill out the PDF version provided below. If you choose to use e-mail, please fill out the PDF form, save it to your computer's desktop, and then send the file as an attachment to the e-mail address below.
